The Site-Specific Health and Safety Plan will be reviewed by the project <br /> manager, field staff and contractors prior to beginning field operations at the site. <br /> Applicable permits for well installation from San Joaquin County and street <br /> encroachment from Caltrans will be obtained prior to drilling. A standard <br /> encroachment permit application has been filled out and will be submitted to Caltrans <br /> upon the PHS/EHD approval of the well location(Attachment C). <br /> The specific scope of work is discussed below: <br /> • Preparation of a site health and safety plan. <br /> • Permitting and installation of one groundwater monitoring well, including <br /> development of the well. The 2-inch diameter well will be located southeast of the <br /> site within a Caltrans easement beneath the elevated highway which trends east-west <br /> in the area south of the site. The purpose of this well is to attempt to delineate the <br /> downgradient extent of gasoline hydrocarbons in soil and groundwater offsite <br /> (Figure 2). <br /> • Upon completion of well installation,the proposed well will be surveyed and <br /> existing site wells will be re-surveyed by a licensed surveyor to mean sea level. <br /> Groundwater gradient will be interpreted utilizing wellhead and water level <br /> elevations. <br /> • Evaluate potential health risk associated with the existing hydrocarbon plume. This <br /> will include evaluation of the following: <br /> 1) sensitive receptors in the immediate area of the site, including <br /> presence of schools,hospitals, or other facilities that may utilize <br /> groundwater for water supply; <br /> 2) well search to determine the existence of nearby water supply wells, <br /> and local groundwater use; <br /> 3) research of other potential nearby offsite sources. <br /> • Preparation of a report summarizing the findings of the above tasks. The report will <br /> include a boring log and well detail for the proposed well, soil and groundwater <br /> analytical results, and chain of custody documentation. <br /> Field and laboratory procedures are presented as Attachment A. <br /> Upon completion of plume definition and further assessment of risk associated with the <br /> hydrocarbon plume at the site,the necessity of remedial feasibility studies will be <br /> evaluated. <br /> 920-078.1 A/W PLAN <br />
