VALDEZ, P.O. Box 2048 Stockton, CA 95201, (209) 948-3656. All work requiring traffic control must apply for a closure number seven days prior to start of work. No closures on HOLIDAYS or WEEKENDS. All work under thisermit must ust meet with the approval of the State Representative. Failure on the Permittee's part to comply with any provision will be cause for revocation of this permit. IMMEDIATELY FOLLOWING COMPLETION OF WORK PERMITTED HEREIN, PERMITTEE SHALL FILL OUT AND MAIL NOTICE OF COMPLETION PROVIDED BY GRANTOR. Permittee's attention is directed to Section 6, "CONTROL OF MATERIALS", of the State Standard Specifications Reference to Engineer in'the State Standard Specifications shall include State Representative. Permittee is directed to Standard Specification section 7-1.11 Preservation of Property, and Business and Professions Code, Section 8771. Permittee shall physically inspect the work site and locate survey monuments prior to work commencement. Monuments shall be referenced or reset in accordance with Business and Professions Code. A pre-job conference with the Permittee, Contractor, and State Representative maybe required prior to start of work. Permittee shall contact the State Representative and arrange the meeting. This meeting may be waived at the discretion of the State Representative. Permittee shall, prior to commencement of any work, provide the State Representative with the name and phone number of the person in responsible charge of the work to be performed under this permit. In accordance with Section 6500 of the Labor Code, a Cal-OSHA permit is required prior to the start of work. The general area in the vicinity of the proposed permit work may be environmentally sensitive. All mitigation and related work, in accordance with. General Provision No. 26, is the responsibility of the Permittee. In addition to the attached General Provisions, the following special provisions are also applicable for installation and monitoring of groundwater monitoring wells: f 1. Unless specifically authorized elsewhere in this permit, survey work shall not be performed: A. On holidays or an afternoon proceeding a holiday. B. In medians. C.
