In addition, San Joaquin County requires that the <br /> consultant submit a Master File Record (MFR)form an encroachment permit issued by the City of Stockton, a <br /> Licensed Contractors' Declaration (LCD)form and the fee for completing the fieldwork. I have appended the <br /> permit application for destroying the wells, the LCD form and the MFR form for you to review.
