Phone: <br /> The following wells were properly destroyed: MW-1, MW-2, MW-3, MW-4, MW-5, 916.985.2079 ext. 31 <br /> MW-6, MW-7, MW-8M, MW-8D, MW-9M, MW-9D, MW-10M, MW-1 OD, MW-11 M, <br /> Email: <br /> MW-11 D, MW-12D, MW-13M and MW-13D. Thomas.Potter <br /> @arcadis-us.com <br /> Destruction of the wells was performed in general accordance with the methodology <br /> presented in the Work Pian for Monitoring Well Destruction(the Work Plan) Our ref: <br /> submitted to the San Joaquin County Environmental Health Department (SJCEHD) GP09BPNA.C137 <br /> on July 27, 2010. The Work Plan was prepared by ARCADIS in response to a <br /> SJCEHD request as noted in its' June 15, 2010 letter. <br /> Pre-Field Activities <br /> Prior to conducting field activities associated with the destruction of the groundwater <br /> monitoring wells, the pre-field tasks described below were completed. <br /> Health and Safety. ARCADIS prepared a site-specific Health and Safety Plan for the <br /> scope of work as required by the Occupational Health and Safety Administration <br /> (OSHA) Standard "Hazardous Waste Operations and Emergency'Response" <br /> guidelines (29 CFR 1910.120).The document was reviewed.and signed by <br /> ARCADIS personnel and subcontractors performing work at the site. <br /> Permitting. ARCADIS secured well destruction permits from the SJCEHD and <br /> encroachment permits (for those wells located in sidewalks, streets or planters) from <br /> the City of Stockton Public Works Department (SPWD).
