Analytical results from the July 1998 UST pit <br /> observation well sampling event evidenced Methyl Tertiary Butyl Ether(MTBE) at 52.4 <br /> parts per billion (ppb) in ground water from one of the wells. <br /> PHS/EHD approves the work plan as submitted. Please submit a boring permit <br /> application and required $89.00 fee to PHS/EHD. Please give 48 hours notice to <br /> PHS/EHD prior to scheduling fieldwork. <br /> According to PHS/EHD records, two monitoring wells were installed on site at the time of <br /> the underground storage tank installation in 1984.
