The following is an itemized list of hazardous materials violations that have not been addressed for GASTELUM AUTO REPAIR as of May16, 2019. Open violations from February 25, 2019 inspection Violation #6 - Failed to complete and/or submit a site map with all required content. The map does not show loading areas, storm and sewer drains, and emergency response equipment(fire extinguishers and spill control). A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and label as"NA". Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. Violation #9 - Failed to annually review and certify that the business plan is complete, accurate, and up to date. The business plan information has not been reviewed and resubmitted in the California Environmental Reporting System (CERS) annually. The hazardous materials inventory shall be submitted by January 15 of each calendar year and may be submitted beginning November 1 of the previous year. Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. Violation#11 - Failed to provide or document initial and/or refresher training to appropriate personnel. Employee training records were not available at the time of inspection. The business plan shall include provisions for ensuring that appropriate personnel receive initial and annual refresher training. This training shall be documented electronically or by hard copy and shall be made available for a minimum of three years. Immediately provide employee training for appropriate personnel and submit a copy of the training records to the EHD. Violation#12 -Failed to have the business plan readily available to facility or EHD personnel. Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency response plans and procedures, the inventory of information required by this article, and the site map required by this section shall be readily available to personnel of the business or the unified program facility with responsibilities for emergency response or training pursuant to this section.
