One permit per <br /> parcel. .a PHS-EHD permit is to match each and every city encroachment <br /> permit, one-to-one. For this phase it looks like there will be two PHS-EHD <br /> permits unless the city issues multiple. Offsite 'well location' in the <br /> first line on the appl is the physical address of the closest <br /> building/parcel to the well, and NOT the UST site's address (unless .the well <br /> is directly in front of the site) . For Kilpatrick, for example, MW-4 across <br /> the street is closest to the glass shop. . .the 'well location' will be the <br /> address of the glass shop. Monitoring wells placed in the City right of way <br /> require both an "encroachment" AND a "revocable"permit and a. bond to be on <br /> file with the Public Works dept. I cannot issue the PHS-EHD well permit <br /> WITHOUT a copy of both of the City permits on file with me. The city takes <br /> about 4 weeks or so, especially if you have incomplete submittals to <br /> them. . .I wouldn't schedule any drilling until the City gives you an idea of <br /> when they can finish your processing. . .I only need 1-2 days once I have all <br /> of the permit copies.
