I have submitted a revocable well permit and
encroachment permit application to the City of Stockton for the offsite
j wells at the subject site. The City received these applications on
12/03/2001. Once we have these approved, I will submit well permit
} applications to the County and we will proceed with the site investigation.
1
-----original Message-----
From: Mike Infurna [mailto:MInfurna@phs.hs.co.san-joaquin.ca.us] 'I
Sent: Thursday, October 18, 2001 10:39 AM -
To: 'mmilward@contactpsc.com'
Subject: San Joaquin County well permit app
enclosed is a master of the actual permit application. The permit- isissued
to the C-57 contractor and is non-transferrable once issued. Return this
completed form with a $89 check made to "PHS-EHD" . One permit per
parcel. .a PHS-EHD permit is to match each and every city encroachment
permit, one-to-one. For this phase it looks like there will be two PHS-EHD
permits unless the city issues multiple. Offsite 'well location' in the
first line on the appl is the physical address of the closest
building/parcel to the well, and NOT the UST site's address (unless .the well
is directly in front of the site) . For Kilpatrick, for example, MW-4 across
the street is closest to the glass shop. . .the 'well location' will be the
address of the glass shop. Monitoring wells placed in the City right of way
require both an "encroachment" AND a "revocable"permit and a. bond to be on
file with the Public Works dept. I cannot issue the PHS-EHD well permit
WITHOUT a copy of both of the City permits on file with me. The city takes
about 4 weeks or so, especially if you have incomplete submittals to
them. . .I wouldn't schedule any drilling until the City gives you an idea of
when they can finish your processing. . .I only need 1-2 days once I have all
of the permit copies. Here's a copy of the LCD for your records. . .Most .
drillers around here have their own masters and once you are ready, have
themfax me a completed one at (209) 46B-3433 .
. «WELL-PERMIT-APPL.doc>> «LCD-contractor declaration +auth to sign.doc
4
t
1
