Center St. <br /> Stockton,CA., 95201 <br /> San Joaquin County Public Health Services, Environmental Health Division (PHS-EHD) has <br /> reviewed the schedule faxed on May 14, 1999 and offers the following comments. <br /> Your proposal to allow another 60 days to receive and evaluate the "bids", select a contractor, and <br /> complete the field work is unacceptable. <br /> On February 8, 1999, PHS-EHD approved the revised work plan to place monitoring wells on and <br /> off-site for the investigation required at this site. The onsite boring permit was issued to Spectrum <br /> and you were advised to contact the City of Stockton, Public Works Department to obtain the <br /> required encroachment and revocable permits. PHS-EHD faxed your consultant a copy of the city's <br /> requirements also. <br /> Prior to receiving this work plan revision, PHS-EHD granted many extensions. PHS-EHD was <br /> assured in phone conversations that every attempt would be made to conduct the required <br /> investigation in a timely manner. <br /> Since February 8, 1999, no progress has been made to enact the approved work plan. In phone <br /> conversations with the City of Stockton's Public Works Department, PHS-EHD has learned that <br /> you have not submitted any of the paperwork needed to obtain the permits. <br /> Since you have failed to take the adequate steps necessary to obtain the encroachment permit or <br /> install the onsite monitoring wells, PHS-EHD hereby provides the following compliance dates. <br /> • Onsite monitoring wells#1 and #3 are to be installed by June 1, 1999. <br /> • Copies of the required city encroachment and revocable permits are to be submitted to PHS- <br /> EHD by June 7, 1999. <br /> • Off-site monitoring well #2 is to be installed by June 21, 1999. <br /> • The report of findings for installation of all three monitoring wells (complete per LUFT Manual <br /> -Tri-Regional Guideline Standards) is to be submitted to PHS-EHD by July 30, 1999. <br /> • Quarterly monitoring is to begin with Third Quarter-1999 (July,August, and September) and the <br /> complete Quarterly Monitoring Report is to be submitted to PHS-EHD by the 15`h of the <br /> month following the quarter's end (October 15, 1999). <br /> Faxes will not be accepted for compliance date requirements. <br /> A Division of San Joaquin County Health Care Services <br />
