Well Destruction Work Plan <br /> SJCEHD Case Number:2371 <br /> Aprii 19,2012 <br /> Page 2 of 5 <br /> PRE-FIELD ACTIVITIES <br /> Upon receipt of SJCEHD approval of this Work Plan, the City and SJCEHD permit applications <br /> will be completed and submitted for approval. The permits will be available on site during the <br /> field activities. PSC will also coordinate schedules with the third party utility, drilling, and <br /> surface restoration subcontractors to perform the various related tasks. Upon receipt of the <br /> permits, PSC will coordinate the field schedule with the subcontractors and the property owner <br /> to for site access to perform the well destruction activities. <br /> PSC will prepare a Health and Safety Plan (HASP) for PSC personnel and their subcontractors <br /> performing in-field activities. The HASP will include a brief summary of the scope of work, all <br /> emergency contact information, and standard operating procedures for work to be performed. <br /> The HASP will also include a job safety analysis (JSA) and all field safety forms necessary for <br /> the scope of work. <br /> PSC will mark the well locations and indicate total distances for utility clearance in white paint <br /> on the street or sidewalk in front of the subject property for the USA state call-in notification. <br /> PSC will call USA to schedule the utility locate upon approval of this Work Plan and receipt of <br /> the approved permits. The SJCEHD will be notified at least 72-hours prior to starting the field <br /> activities. <br /> IN-FIELD ACTIVITIES <br /> During in-field activities, safety meetings will be held prior to starting field activities each day. <br /> All PSC and subcontractor personnel will attend the meetings, and/or be provided copies of the <br /> HASP and appropriate JSAs when they arrive on site. All personnel will be informed of the <br /> health and safety concerns regarding the work to be performed and the site conditions. The <br /> HASP and JSAs will be signed before commencing and after completion of work on the site each <br /> day. <br /> Subdynamic Utility Detection Services, Inc. (SubDynamic) will perform the third party utility <br /> location clearance prior to starting the field activities. Subdynamic will meet with PSC and the <br /> drilling personnel on the morning that the field activities will commence. All essential utilities <br /> will be identified within the specific drilling locations and drilling shall not commence until <br /> clearance has been assured by SubDynamic. Since the borehole locations are existing wells, the <br /> additional clearance activities of hand augering or using an air knife are not required. If wells <br /> are located in close proximity to subsurface utilities, an air knife or hand auger may be required <br /> or alternatively the well may be pressure grouted in place. <br /> Wells MW-2, MW-2A, SP-1, and VW-1 are located in the sidewalk area just in front of 1717 <br /> South Center Street. Well MW-4 is located in the parking lane on the east side of Center Street <br /> just in front of 1725 South El Dorado Street. Stanley Engineering will perform the traffic safety <br /> control for both areas. Traffic control maps for each area are presented on Figures 4 and 5, <br /> respectively. <br /> The concrete and/or asphalt surface may need to be pre-cored in preparation for the well <br /> destruction borings.
