I cannot issue the permit because I need an additional $366 for the inspection fee as well as the <br /> completed Master File Record Information Form (MFR)and the correct Well Permit Application. The latest <br /> versions of the MFR and the Well Permit Application form can be obtained by visiting EHD's website at <br /> www.si.gpv.org/ehd.
