Complete and submit a copy of Underground Storage Tank Certification of <br />Installation/Modification (Former Form C) to this office no later than 30 days <br />after work is completed. <br />6. Written secondary containment test results must be submitted within 30 days of <br />completion of the test, to this office, in order for EHD to consider secondary <br />containment testing complete. <br />7. Permit expires 180 days from date of approval.
