REHS <br />PROGRAM COORDINATORS <br />Robert McClellon, REHS <br />Jeff Carruesco, REHS, RDI <br />Kasey Foley, REHS <br />flit <br />cc,y <br />SR0060366 <br />The San Joaquin County Environmental Health Department (EHD) has completed the review of <br />the application for a permit to perform a cold start on the monitoring panel after replacement of <br />the CPU board on the underground storage tank (UST) system at the above noted address. <br />The permit has been approved with the following conditions: <br />Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Stacy Rivera at (209) 468-3440 to arrange the following inspection <br />appointments (a 48-hour notice is required for inspection appointments). Please be <br />aware that inspections for emergency work must be scheduled within five days of permit <br />approval. <br />a. Sensor functionality: Functionality of the monitoring system must be verified to <br />ensure that monitoring has been restored to meet the requirements of Title 23 <br />California Code of Regulations and the Health & Safety Code. A copy of the alarm <br />history shall be printed prior to performing the "cold start" of the Veeder Root system <br />and made available at the time of EHD inspection. <br />Please be aware that the facility cannot operate the UST system or receive fuel in <br />the tanks prior to the EHD inspection and approval. <br />4. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all of the <br />following: <br />a. One of several permissible valid, current contractor's licenses issued by the <br />Contractors State License Board (may be issued to the installation company), or a <br />Tank Tester's license issued to the individual by the State Water Board, and <br />
