No person may own or operate an UST unless a permit for its operation <br />has been issued by the local agency to the owner or operator of the UST system. Immediately obtain a permit to operate a <br />UST system from the EHD. An "Operating without a Permit" penalty applies. <br />This is a Class I violation. <br />#109 CCR 2637 Failed to test secondary containment <br />OBSERVATIONS: Secondary containment testing was performed on September 20, 2017 and the 87 secondary product, <br />91 secondary product, diesel secondary product, and all UDCs failed. To date, no repairs have been made. Secondary <br />containment testing is required once every 36 months. Immediately have a properly licensed, trained, and certified <br />contractor repair or replace the failed component under permit and inspection of the EHD. <br />This is a Class II violation. <br />OVERALL NOTES AND COMMENTS <br />Immediately complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork. <br />Note: A secondary containment testing must be conducted every 36 months.
