The designated operator failed to check and note the correct test dates for the annual monitoring <br /> system certification, line leak detector and spill bucket testing on the designated operator monthly <br /> inspection reports from August 2012 to December 2012, five monthly reports. Designated operator had <br /> documented the incorrect date of 06/01/12 rather than the correct date of 06/05/12 which was corrected <br /> in the monthly reports from January 2013 to present. During the monthly inspection, the designated <br /> operator shall check that all required testing and maintenance for the UST system have been completed <br /> and document the dates they were done. Ensure that designated operators performing monthly <br /> inspections at this facility are including all of the required information on the reports. Submit a written <br /> statement how to ensure reports will have correct dates. <br /> Submit to SJC EHD within 30 days (07/12/13): <br /> 1. one copy of the completed "Return to Compliance Certification" form <br /> 2. a written corrective action statement ensure reports will have correct test dates <br /> Notes: <br /> -CER& Underground Storage Tanks: lists incorrect secondary product piping construction. <br /> Monitoring Plan in Section X.
