The permit has been approved with the following <br /> conditions: <br /> 1. Contact the local fire department Local Planning or Building Department, and the <br /> San Joaquin Valley Air Pollution Control District for any applicable permits and <br /> requirements. <br /> 2. Contact Michelle Henry at (209) 953-7699 to arrange the following inspection <br /> appointment (a 48-hour notice is required for inspection appointments): <br /> a. Functionality test to ensure that monitoring has been restored to meet the <br /> requirements of California Code of Regulations Title 23 and the Health & <br /> Safety Code. <br /> 3. Any changes in information and scope of work must be submitted to EHD for <br /> approval prior to commencing work. All new addenda supersede the previous <br /> ones. <br /> 4. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all <br /> of the following: <br /> a. One of several permissible valid, current contractor's licenses issued by <br /> the Contractors State License Board (may be issued to the installation <br /> company), and <br /> b. A valid, current California UST Service Technician certificate issued by <br /> the International Code Council (ICC), and <br /> c. A valid, current certificate of training from the manufacturer(s) of the <br /> component(s) being tested, repaired, or serviced, <br /> and <br /> d. Each person installing or testing any component must hold a certificate of <br /> training from the manufacturer of that component. <br />
