Other pertinent info, if any: Between August 13, 2014 and October 6, 2014, MLTR Company allowed the <br /> transportation offour loads of waste tires to be delivered to a location not authorized by CalRecycle <br /> which is a violation of PRC 42951(b). The location is Used Tire Wholesale(UTW)located at 3650 <br /> Enterprise Avenue, Hayward, CA [Tire Program Identification(TPID)number 17814541. This site in not <br /> authorized to accept and store more than 499 waste tires without a Waste Tire Facility permit. <br /> On August 13, 2014, ML TR Company allowed the transportation of one load of 518 waste tires to he <br /> delivered to UTW. On October 1, 2014, ML TR Company allowed the transportation of one load of 527 <br /> waste tires to UTW. Lastly, on October 6, 2014, ML TR Company allowed the transportation of two loads <br /> of waste tires: 665 waste tires and 529 waste tires to UTW. <br /> At the time of initial registration and subsequent yearly renewals, the waste tire hauler is provided with a <br /> copy of Title 14, California Code of Regulation (CCR)relating to the transportation and manifesting of <br /> waste/used tires. In these regulations, it clearly states that the hauler shall not deliver waste tires to a <br /> location not legally authorized to accept waste tires[14 CCR§18460.2(1)].Authorized locations are <br /> permitted majorhninor waste tire facilities(WTF),permitted solid waste facilities,port locations, <br /> approved collection sites as defined by Title 14, California Code of Regulations, section 17225.717, or <br /> locations that have less than 500 waste tires. The hauler had been penalized in 2012, therefore these will <br /> be second level penalties. <br /> For Late Renewals Haulers <br /> 1. When was the renewal processed: <br /> 2. How many times in the past were they late in renewing: <br /> 3. When did the hauler first become registered: <br /> 4. Other pertinent info, if any: <br /> For Unregistered Haulers <br /> 1. How many unregistered hauler complaints have they had in WTMS: <br /> 2. Was the hauler sent an LOV by certified mail:❑ Yes ❑No <br /> 3. Were there any inspection reports in WTMS documenting the type of violation noted above? <br /> ❑Yes ❑No; if yes include: <br /> 4. For haulers who's total numbers of CTL's exceed 500 during the last <br /> 6 month time period, take the last 400 forms and follow the instructions below: <br /> 2. Export CTLs to an Excel spreadsheet <br /> 3. Remove excess/duplicate forms on the spreadsheet, in other words,only I form regardless/row many receipts <br /> 4. Determine the total forms for that time period <br /> 5. Determine the total number offorms that contain error for that time period. This can be done by looking at the CTL <br /> Form LOV Error Report in GM,1S for the same titre frame(ex. 6 months.)Review each log that 6ir1'A1S has indicated <br /> has an error and assess whether or not it is a true error and keep track of the total number you determine to have <br /> errors. <br /> 6. Divide the total number of errors by the total forms to get a percentage. If the percentage is more than 10%proceed to <br /> next step, if it is less than 10%send another LOV to attempt compliance. If after the second LOV the percentage is still <br /> above 2%discuss with Supervisor. <br /> 7. If the total number offorms were less than 20 forms take the total number of errors and we will only fine them for 25% <br /> of the errors found in those<20 forms(so that we may be fair to the small haulers). If there are 20 or more forms then <br /> proceed to the next step. <br /> 8. Take the total number offorms and multiple it by 25%to obtain your research number.
