($10, 000. 00) DOLLARS for property damage. The policy of <br /> insurance so provided shall contain a contractual liability <br /> endorsement covering the liability assumed by the Permittee by <br /> the terms of his permit and shall contain a provision that <br /> such policy may not be cancelled except after thirty (30) days <br /> notice in writing to be given to the Public Works Director. <br /> The copies of these policies or certificates evidencing the <br /> same shall be filed with the Public Works Director of the City <br /> after approval of the City Attorney. <br /> 5. The issuance of this permit by the City does not create any <br /> agency on the part of the permittee, and the permittee will <br /> hold the City of Stockton harmless for any action for damages <br /> arising out of the permittee's activities performed under this <br /> permit. <br /> 6. Permittee, or his contractor, shall obtain an Encroachment <br /> Permit for the installation of the well(s) . <br /> 7. The completed well head shall be covered with a meter box or <br /> valve box as approved by the City of Stockton. The top of the <br /> box shall be flush with the surrounding ground or pavement. <br /> 8. The provisions of the State of California, Department of Water <br /> Resources Bulletin 74-81 entitled "Water Well Standards: State <br /> of California" shall be applicable to this permit. <br /> 9. All drilling locations, methods, and well design are to be <br /> approved by the California Department of Health Services and <br /> the Central Valley Regional Water Quality Control Board. <br /> 10 . Drilling operations that create loud noise shall be confined <br /> between the hours of 7 a.m. and 6 p.m. <br /> 11. Portable mud tanks are to be used. <br /> 12 . Disposal of water from the well development shall be in <br /> accordance with current federal regulations. <br /> 13 . Blocking or barricading City streets shall be subject to the <br /> approval of the City Traffic Department. Traffic cones and <br /> warning signs shall be used to direct traffic. <br /> 14 . Upon completion of water monitoring, the well shall be <br /> abandoned in accordance with the requirements of the San <br /> Joaquin Local Health District. <br /> PUBLIC WORKS DIRECTOR <br /> CITY OF STOCKTON <br /> 2 <br />
