Stantec <br /> Well Restoration and Well Destruction Report <br /> Stockton Charter Way Commingled Plume No. CP0057 <br /> November 15, 2011 <br /> 2.0 PERFORMED SCOPE OF WORK <br /> 2.1 Site-Specific Health and Safety Plan <br /> As required by the OSHA "Hazardous Waste Operations and Emergency Response" guidelines <br /> (29 CFR 1910.120) and by California Occupational Health and Safety Administration <br /> (Cal-OSHA) "Hazardous Waste Operations and Emergency Response" guidelines (CCR Title 8, <br /> Section 5192), Stantec prepared a Site-specific Health and Safety Plan (HASP). At a minimum, <br /> the HASP defined the proposed field activities, described physical and chemical hazards that <br /> may be associated with the work, provided a map to the nearest emergency medical facility, and <br /> included material safety data sheets for hazardous chemicals that were used or produced <br /> during the work. A copy of the HASP have been available on-site at all times during field work. <br /> The field staff and subcontractors performing field activities have reviewed the HASP prior to <br /> beginning field operations at the Site. <br /> 2.2 Permitting <br /> Prior to well restoration activities, Stantec obtained well permits for restoring monitoring wells <br /> SMW20-83.5 and SMW-23 from the San Joaquin County Environmental Health Department <br /> (SJCEHD). Stantec also obtained well permits for SMW-7, SMW-18-80, and SMW-18-120 prior <br /> to well destruction activities. <br /> 2.3 Preliminary Field Activities <br /> On September 20, 2011, Stantec staff marked the monitoring well locations for Underground <br /> Service Alert (USA), notified USA North to identify underground facilities near the monitoring <br /> well locations, and notified the station manager regarding the well restoration and well <br /> destruction activities. On September 26, 2011, Stantec staff supervised Cruz Brothers Locators, <br /> of Santa Cruz, California (Cruz Brothers) conduct an underground survey to locate monitoring <br /> wells SMW20-83.5 and SMW-23 and identify underground facilities near monitoring wells <br /> SMW-7, SMW-18-80, and SMW-18-120. A station light underground wiring and conduit was <br /> identified approximately two and a half feet from well SMW-7. <br /> 2.4 Well Restoration Field Activities <br /> In preparation for the property redevelopment located at 378 West Charter Way east of former <br /> Texaco (Shell), WDC had previously lowered off-site wells SMW20-83.5 and SMW-23 by air- <br /> knifing the well borehole to approximately three feet below ground surface (bgs), cutting and <br /> capping each well extending above the air-knifed depth, covering with Morrison well vaults, and <br /> backfilling with sand and soil. The property located at 378 West Charter Way later had been <br /> redeveloped as a commercial store with the building and driveway in front of the property <br /> adjacent to West Charter Way and customer parking behind the store building. <br /> Cruz Brothers marked the location of wells SMW20-83.5 and SMW-23 on the driveway leading <br /> to the customer parking behind the store building. On October 4, 2011, WDC jackhammered <br /> and hand-dug the marked locations in attempt to exposed the previously lowered well SMW-23, <br /> Stockton Charter Way CP-Well Restoration and Well Destruction Report.doc 2 <br />
