Stantec <br /> Well Restoration and Well Destruction Report <br /> Stockton Charter Way Commingled Plume No. CP0057 <br /> November 15, 2011 <br /> On Friday October 7, 2011, at 6pm PDT, Stantec was overseeing a sonic drill crew preparing for <br /> demobilization, when a hydraulic hose failed releasing approximately 1.5 to 2.0 gallons of <br /> synthetic oil onto visqueen located beneath the sonic drill rig. The field crew initiated stop work <br /> and immediately contained the release with absorbent pads, kitty litter, and absorbent socks <br /> that were readily available inside the exclusion zone. No synthetic oil reached the ground <br /> surface or the destroyed well. At the time of the hydraulic hose failure, the sonic rig operator <br /> was adding slack to the fast line located on the boom, while the sonic rig was running on idle. <br /> The sonic rig operator had already lowered the mast and raised the outriggers. The sonic drill <br /> crew had destroyed and grouted SMW-18, but not completed the boring to surface. <br /> Field notes during the well restoration and destruction activities are included as Appendix B. <br /> 2.6 Waste Disposal <br /> WDC containerized soil cuttings, drilling fluids, and construction debris generated during well <br /> restoration and well destruction activities into Department of Transportation-approved 55-gallon <br /> drums and temporarily placed on the southeast corner of the former Texaco (Shell) located at <br /> 440 West Charter Way, near the existing trash enclosure pending characterization and disposal. <br /> A total of 26 drums have been generated during the well restoration and well destruction <br /> activities (12 contain soil, nine contain fluids, two contain construction debris, and three contain <br /> spill response material). Belshire Environmental Services, Inc. of Foothill Ranch in California <br /> removed the drums on October 21, 2011. Waste manifest record will be submitted in the Semi- <br /> Annual Monitoring Report Fourth Quarter 2011 report. <br /> 2.7 Survey Data <br /> Stantec will surveyed wells SMW-20-83.5 and SMW-23 for latitudes, longitudes, and elevations <br /> using Vertical Datum NAVD 88 in US survey feet at the former Texaco (Shell) in fourth quarter <br /> 2011.
