ACCESS AGREEMENT Well Destructions 721 East Charter Way (a.k.a. East Dr. Martin Luther King Jr. Blvd), Stockton, CA (APN: 147-343-10) We, Kee Sun and Mei Au,the property owners of 147-343-10„ do hereby grant permission to Advanced GeoEnvironmental, Inc. (AGE) on behalf of the responsible party, Swaran Chouhan, to destroy one (1) groundwater monitoring wells under San Joaquin County Environmental Health Department permit at 721 East Charter Way, Stockton, California. The site under investigation is located adjacent to the above referenced property at 701 East Charter Way, Stockton, California. The proposed work is being performed under the directive of the California Regional Water Quality Control Board - Central Valley Region. The scope of work will include the following: • Destruction of one (1), 2-inch diameter groundwater monitoring wells. The well will be drilled-out to one foot deeper then its installed total depths utilizing 8.25-inch diameter hollow-stem augers.All original well construction materials will be removed (including well casing, screen, filter pack, and annular seal) to ensure that no residual contamination is sealed within the abandoned borehole. Remaining boreholes will be backfilled to surface grade with neat cement and covered with concrete or asphalt, depending on the surrounding surface material. • The surface will be returned to the original condition. All expenses for the well destructions performed at 721 East Charter Way, Stockton, California shall be paid by the responsible party (Swaran Chouhan), and shall be conducted under permit and in accordance with local regulations. Reporting of the proposed work at your property will be forwarded to you the property owner.
