I have reviewed the submittals and have found the following issues: <br /> * Violation#5, Inventory not complete. <br /> New oil—the tank of new oil has not been reported. <br /> * Violation#6, the site map is not complete. <br /> The site map does not the location of the emergency shutoff, emergency response <br /> equipment (spill control), and the tank of new oil. A site map shall contain a north <br /> orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br /> access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment. If a site <br /> map element is not applicable for your facility then list it on the map and label as <br /> "NA". . <br /> * Violation#8 - The Employee Training Plan has not been submitted. <br /> * Violation#11 - Documentation of employee training has not been received (do not upload <br /> to CERS). <br /> As a reminder, the comply by date of November 30, 2017 has passed; it is the policy of the <br /> EHD to send a past due notice once the violation due date has passed; all time spent attempting <br /> to obtain compliance may be billed at $152 an hour. <br /> As of September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152).
