J Hunter and Associates, dated June 1993. <br /> PHS/EHD concurs with the reconimendanons and conclusions presented in chis report in that soli at this <br /> site does not require any further removal or remediation. Contaminated soil has been transported to the <br /> Forward Landfill and receipts for this have been received by PHS/EHD <br /> Monitoring wells #1 and #2 require continued monitoring for off-site contaminant migration. <br /> Pursuant to Section 2652(d) of the Underground Storage Tank Regulations (Califomia Code of Regulations, <br /> •� Title 23, Chapter 16) a quarterly monitoring report shall be submitted to Public Health Services, <br /> Environmental Health Division and Central Valley Regional Water Quality Control Board (CVRWQCS) until <br /> the investigation and cleanup of the site is deemed adequate The report is to show The <br /> -status of investigation and cleanup activities including the results of all investigations <br /> completed to date, <br /> -proposed continuing or next phase of investigation; <br /> -method of cleanup proposed or implemented, schedules for the completion of the <br /> investigation of the site and remediation; <br /> -method and location of disposal of the released hazardous substance and any <br /> contaminated soil, groundwater or surface water; <br /> -manifest required for transport of hazardous substances; <br /> -monitoring well data including groundwater elevation, pH, conductivity, gradient, <br /> .analytical results of groundwater or soil sampling; and <br /> -site map showing the "zero line" of contamination, if known, and changes m the above <br /> measurements over the last quarter <br /> If you have any questions, contact Michael Collins, REHS, of my staff at (209) 468-0338. <br /> Jogi Khanna, M.D , NQ I-i.
