The following is an itemized list of hazardous materials violations that have not been <br /> addressed for LINEAGE LOGISTICS as of March 05, 2019. <br /> Open violations from March 07, 2016 inspection <br /> Violation #5 - Failed to complete and/or submit hazardous materials inventory for all reportable materials on <br /> site. <br /> Reportable amounts of anhydrous ammonia, lead acid batteries, a 55 gallon drum of waste oil, a 55 gallon drum of <br /> propylene glycol, and a 55 gallon drum of BromMax, two 55 gallon drums of Freeze Zone detergent and a 249 c.f. <br /> cylinder of compressed oxygen were found on site that have not been reported for 2016 Any material that meets or <br /> exceeds the reportable quantity shall be reported in the facility's business plan. Immediately log into the California <br /> Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, add the materials to the hazardous material <br /> inventory, and submit to the EHD for approval. <br /> The Hazardous Materials Inventory form submitted last year for batteries wrongly identified the amount of batteries <br /> in pounds. When listing batteries in the inventory, the amount of electrolyte must be identified in gallons. This <br /> information must be current and submitted as part of the business plan. Immediately log into the California <br /> Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the correct or updated information, and <br /> submit to the EHD for approval. <br /> Violation#6 - Failed to complete and/or submit a site map with all required content. <br /> A site map was not completed or submitted as part of the business plan for 2016. A site map shall contain a north <br /> orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, <br /> emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency <br /> response equipment.
