The following is an itemized list of hazardous materials violations that have not been <br /> addressed for FTG CONSTRUCTION MATERIALS INC as of March 05, 2019. <br /> Open violations from January 17, 2019 inspection <br /> Violation #5 - Failed to complete and/or submit hazardous materials inventory for all reportable materials on <br /> site. <br /> The Hazardous Materials Inventory form the diesel fuel wrongly identify the storage pressure and storage <br /> temperature as "below ambient' This information must be current and submitted as part of the business plan. <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the <br /> correct or updated information, and submit to the EHD for approval. <br /> Violation#6 - Failed to complete and/or submit a site map with all required content. <br /> The map does not show north orientation (one of the two maps), loading areas, internal roads, adjacent streets, <br /> storm and sewer drains, access and exit points, emergency shutoffs, and hazardous material handling and storage <br /> areas (oxygen and acetylene). A site map shall contain a north orientation, loading areas, internal roads, adjacent <br /> streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment. If a site map element is not applicable <br /> for your facility then list it on the map and label as"NA". Immediately log into the California Environmental Reporting <br /> System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for <br /> approval. <br /> Violation#9 - Failed to annually review and certify that the business plan is complete, accurate, and up to <br /> date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS) annually, by Janyary 15. The hazardous materials inventory shall be submitted by January 15 of <br /> each calendar year and may be submitted beginning November 1 of the previous year. Immediately log into the <br /> CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> Open violations from March 18, 2016 inspection <br /> Violation #6 -Failed to complete and/or submit a site map with all required content. <br /> The site map is not legible. A site map shall contain a north orientation, loading areas, internal roads, adjacent <br /> streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment. Immediately log into the California <br /> Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated information, <br /> and submit to the EHD for approval. <br /> Page 1 of 1 <br />
