The following forms and appropriate fees are enclosed <br /> o Application: Environmental Health Permit/Services for work plan review. <br /> A$89 fee is enclosed with a request for the county to review the work plan. <br /> i <br /> o Application for county boring permit: The application has been signed by a <br /> Kleinfelder staff geologist. <br /> o A Health and Safety Plan for the proposed work <br /> The client will be forwarding an Authorization to Release form and a letter of <br /> acknowledgement.
