Please clarify 1 thru 4 below or send correct documents for your site. <br /> 1 . Underground Storage Tank Unauthorized Release (LEAK) / Contamination Site Report <br /> 2. All permits submitted (except one dated 1988 which is for 1045). <br /> 3 . San Joaquin County Environmental Health Departments directive letter dated 6/4/03 <br /> 4. The Map indicates site address to be 1001 <br /> • On page three (3) of your claim application, be specific with date (dd/mm/yy) . <br /> • Geo Tracker shows another site address as 939 & 1045 Charter Way, Stockton, which we have a <br /> claim application for (Claim #552-Nomellini Construction Company), which had a release in 1988 <br /> and 1989, and received closure in 1996. Is this the same site address the same location as your site? <br /> • Claimant is required to have current financial responsibility documents on file. Enclosed is a <br /> Financial Responsibility Guide that explains the requirements . The Certification of Financial <br /> Responsibility form and Chief Financial Officer Letter form are tabbed for your convenience and <br /> must be completed along with the applicable supporting documents. Please submit the original <br /> documents to the local regulatory agency and forward a copy to the Fund. <br /> • Claimant is requesting priority class "C" and must submit documentation verifying that claimant does <br /> not employ more than 500 full time or part time employees.
