The consolidated manifest receipt for last 3 <br /> years were not available for inspection. All copies of each receipt shall contain all of the following information: <br /> -The name, address, identification number, contact person, and telephone number of the generator, and the <br /> signature of the generator or the generator's representative; <br /> -The date of the shipment; <br /> -The manifest number; <br /> -The volume or quantity of each waste stream received, its California and RCRA waste codes, the waste stream <br /> type listed in subdivision (c), and its proper shipping description, including the hazardous class and United Nations/North <br /> America (UN/NA) identification number, if applicable; <br /> -The name, address, and identification number of the authorized facility to which the hazardous waste will be <br /> transported; <br /> -The transporter's name, address, and identification number, <br /> -The driver's signature, <br /> -A statement, signed by the generator, certifying that the generator has established a program to reduce the volume <br /> or quantity and toxicity of the hazardous waste to the degree, as determined by the generator, to be economically <br /> practicable. <br /> Ensure that all future receipts for hazardous waste shipments using a consolidated manifesting procedure contain all <br /> required information. This is a Minor Violation. <br /> 117 Failed to report program data electronically. A submission to the California Electronic Reporting System (CERS) for the <br /> hazardous waste program is not complete (site map still has to be submitted). Beginning January 1, 2013, all <br /> businesses are required to submit all new (or any changes to existing) hazardous waste information online to the CERS <br /> at http://cers.calepa.ca.gov. Be sure to include your hazardous waste activity in the Businesses Activities section in <br /> CERS in addition to any other relevant activities and required fields. This is a Minor Violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by November 6, 2014. <br /> Consent to perform the inspection, take photos and collect samples was given by Lucio Palacios the owner. <br /> Facility generates hazardous waste from painting and cleaning of equipment using degreaser/solvent, and <br /> occasional auto repair waste when servicing company vehicles on site. Business has been in operation the last <br /> 12 years. Owner does not know how hazardous waste was disposed in the past since his daughter took care of it <br /> and she no longer works for him. Based on inspection and interview, the facility generates less than 5 tons of <br /> hazardous waste in the last calendar (Jan-Dec). <br /> Observed facility disposed 1 gallon-jug and 4 quart-bottles of used oil, a spent lead acid battery, and 2 drained <br /> used oil filters at a local oil collection center (Autozone on Wilson Way). Discrepancy has been corrected on site. <br /> Observed facility posted emergency coordinator information and modified contingency plan on site.
