Copies of the monthly and annual inspections <br /> required by the SPCC plan and the S7"I SP-001 standard were not found Inspections and tests must be conducted <br /> in accordance with the written procedures developed In the Spill Prevention, Control, and Countermeasure(SPCC) <br /> Plan. Records of these inspect,'ons and tests must be signed by the appropriate supervisor or inspector and kept on <br /> site with the SPCC Plan for a period of three years_ hnmedlately locate a copy of all inspection and testing records <br /> for the last three years, maintain them on site, and submit copies to the EHD <br /> This Is a Class 11 violation. <br /> 705 CFR 112.8(c)(1)Container not compatible with oil stored and/or storage conditions. <br /> The SPCC plan did not Include a discussion on this section. Containers shall be compatible with the material stored <br /> and the conditions of storage such as pressure and temperature. The SPCC plan should address this section, cs <br /> provide equivalence as allowed by CFR 112,7(a)(2). <br /> This is a Class 11 violation <br /> CFR 112.8(c)(2)Failed to provide and maintain adequate secondary containment. <br /> The 55 gallon drums by the outer fence,the 240 gallon used oil tank and the 5.5 gallon oil drum within the shop area <br /> were observed with insufficient secondary containment. The 55 gallon drums by the anter fence were labeled and <br /> described as staring hydraulic oil,the secondary containment structure Inas full of liquid and unable to contain the <br /> contents of the largest conaunec The 240 gallon waste oil lank appeared to be single walled and secondary <br /> containment was not evident.The 55 gallon In the shop area is described in the SPCC plan as having secondary <br /> containment for 10%of the contents. All bulk storage tanks must be provided with a secondary means of <br /> containment for the entire capacity of the tank and sufficient freeboard to contain precipitation. Immediately provide <br /> sufficient secondary containment for'this and all otli tanks at this facility. <br /> This is a Class II violation. <br /> 711 CFR 112.8(c)(6)Failed to perform scheduled tank tests or inspections by appropriately qualified personnel. <br /> The 10,000 gallon diesel tank was tested by a certified inspector on 4H 0/2015.The report could not be found <br /> during the time of the inspection. Each aboveground container shell be tested and inspected for integrity on a <br /> regular schedule and whenever repairs are made. The qualifications of personnel performing tests and Inspections, <br /> frequency and type of testing and inspectlons that take Into boronnt container size, configuration, and design shall <br /> be defemlined in accordance with Industry standards. Examples of these integnty tests include, but are not limited <br /> to:visual inspection, hydrostatic testing, radiographic testing, ultrasonic test ng, acoustic amissions testing, 0rother <br /> systems of non-destructive testing. Comparison records and other records of Inspections and tests most be <br /> maintained on site.
