Copies of inspection and testing records were not found on site. Copies of the monthly and annual inspections required by the SPCC plan and the STI SP-001 standard were not found. Inspections and tests must be conducted in accordance with the written procedures developed in the Spill Prevention, Control, and Countermeasure (SPCC) Plan. Records of these inspections and tests must be signed by the appropriate supervisor or inspector and kept on site with the SPCC Plan for a period of three years. Immediately locate a copy of all inspection and testing records for the last three years, maintain them on site, and submit copies to the EHD. This is a Class II violation. 7n5 CFR 112.8(c)(1)Container not compatible with oil stored and/or storage conditions. The SPCC plan did not include a discussion on this section. Containers shall be compatible with the material stored and the conditions of storage such as pressure and temperature. The SPCC plan should address this section, or provide equivalence as allowed by CFR 112.7(a)(2). This is a Class II violation. 706 CFR 112.8(c)(2)Failed to provide and maintain adequate secondary containment. The 55 gallon drums by the outer fence, the 240 gallon used oil tank and the 55 gallon oil drum within the shop area were observed with insufficient secondary containment. The 55 gallon drums by the outer fence were labeled and described as storing hydraulic oil,the secondary containment structure was full of liquid and unable to contain the contents of the largest container. The 240 gallon waste oil tank appeared to be single walled and secondary containment was not evident. The 55 gallon in the shop area is described in the SPCC plan as having secondary containment for 10%of the contents. All bulk storage tanks must be provided with a secondary means of containment for the entire capacity of the tank and sufficient freeboard to contain precipitation. Immediately provide sufficient secondary containment for this and all other tanks at this facility. This is a Class II violation. 711 CFR 112.8(c)(6)Failed to perform scheduled tank tests or inspections by appropriately qualified personnel. The 10,000 gallon diesel tank was tested by a certified inspector on 4/10/2015. The report could not be found during the time of the inspection. Each aboveground container shall be tested and inspected for integrity on a regular schedule and whenever repairs are made. The qualifications of personnel performing tests and inspections, frequency and type of testing and inspections that take into account container size, configuration, and design shall be determined in accordance with industry standards. Examples of these integrity tests include, but are not limited to:visual inspection, hydrostatic testing, radiographic testing, ultrasonic testing, acoustic emissions testing, or other systems of non-destructive testing. Comparison records and other records of inspections and tests must be maintained on site. Immediately conduct the necessary testing and submit a copy of the test results to the EHD, or provide equivalence as allowed by CFR 112.7(a)(2). This is a Class II violation.
