COMPLAINANT WITNESSED <br /> EMPLOYEES TAKING CEILING TILES OFF ABOUT A MONTH AGO DURING WORKING HOURS TO SCRUB RAT <br /> DROPPINGS. COMPLAINANTS SPOUSE WENT TO THE RESTAURANT ON 2/6/2016 AND WITNESSED A <br /> COCKROACH IN THE TO GO BOX THAT WAS HANDED TO THE COMPLAINANTS SPOUSE. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> I spoke to Ben who is person in charge. <br /> Ben was aware of the"cockroach in to go box" part of complaint. <br /> I looked at to go boxes and Ben opened a few, and I did not observe cockroaches at this time. <br /> Per Ben facility is serviced for cockroaches 2 times per month. <br /> I observed grease accumulation on flooring around cooking equipment.
