Time In: 11:01 am <br /> Time Out: 11:30 am <br /> Food Program Complaint Inspection Report <br /> Name of Facility: TRACY PAVILLION GRF2 LLC Date: 12/23/2019 <br /> Address: 2439 NAGLEE RD, TRACY 95304 <br /> Owner/Operator: HUSSEIN, MOHAMMAD Telephone: (209)832-9232 <br /> Program Element: 1600-FOOD PROGRAM Complaint#: C00051353 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> C00051353 states: :COMPLAINANT PURCHASED A BOWL WITH RICE, CHICKEN, ETC.AND A SIDE OF BEANS; <br /> SAYS HE BIT DOWN ON A ROCK AND FRACTURED A TOOTH." <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> EHD Program Coordinator called and spoke to complainant. Per complainant, he ate chicken, gyro meat, mixed beans,feta <br /> cheese and olives. <br /> Owner is on site and is aware of the complaint. <br /> Per owner, chicken is received fresh and is boneless.When assembling the chicken thighs onto the skewer for the vertical <br /> broiler, every chicken thigh is cut four times to make it thin for stacking. Per owner, pieces of cartilege is observed at times <br /> and removed, but no bones. <br /> The mixed bean salad is composed of canned corn, canned kidney beans, canned black beans, diced red onion and spices. <br /> Cans are drained and rinsed prior to mixing the salad together. <br /> Feta cheese and olives are in the Greek salad. The Greek salad is composed of diced cucumber,diced tomato, diced red <br /> onion, dressing,feta cheese and Kalamata olives. Olives are received pitted and sliced. <br /> Rice is washed/rinsed prior to cooking. <br /> Upon inspection this date, no hard objects were observed in any of the food items this date. <br /> Routine inspection conducted at time of complaint investigation.
