Manifest numbers 006068662JJK (10/16/09), 006068663JJK (10/16/09), and <br />007883508JJK (10/27/10) were not found on site. Hazardous waste generators shall retain copies of all <br />manifests for three years or until a copy signed off by the destination facility is received. The signed off <br />copy shall be retained and made readily available for review for three years. If the generator did not <br />receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to <br />which the generator's waste was submitted within 35 days of the date the waste was accepted by the <br />initial transporter, the generator shall contact the transporter and/or the owner or operator of the <br />designated facility to determine the status of the waste. If the generator did not receive a copy of the <br />signed off copy of the manifest within 45 days of the date that the waste was accepted by the initial <br />transporter, the generator shall submit an Exception Report that includes a legible copy of the missing <br />manifest, and a cover letter signed by the generator explaining the efforts to locate the hazardous waste <br />and the results of those efforts. Exception reports shall be submitted to: <br />DTSC Report Repository <br />Generator Information Services Section <br />P.O. Box 806 <br />Sacramento, CA 95812-0806 <br />Immediately locate a copy of the missing manifests or prepare and submit an exception report to DTSC. <br />By October 27, 2012, submit to the EHD a copy of the signed off copy of the manifests or a copy of the <br />exception report submitted to DTSC. <br />REPEAT VIOLATION <br />601. A 55 gallon white steel drum of metal fines was observed in the sanding room with an accumulation <br />start date of October 3, 2011. Facilities who generate 1000 kg or more of hazardous waste per month <br />and or exceed 6000 kg of waste stored on site at any time may not store waste on site for longer than 90 <br />days. Immediately contact a licensed hazardous waste hauler to dispose of this waste under manifest <br />and submit a copy of the manifest to the EHD by October 27, 2012. <br />REPEAT VIOLATION <br />Notice to Comply - Minor violation: (804) <br />804. Used fluorescent bulbs were observed stored in an open container. Universal waste lamps shall be <br />contained in a closed container or package that is structurally sound, adequate to prevent breakage, and <br />compatible with the contents of the lamp.
