(209) 951-9523 <br /> Program Element: 1625 - RESTAURANT/BAR 51-100 SEATS Complaint#: COY7SMKCZ <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> (c) stated: I experienced E. coli food poisoning symptoms after eating at Carls Jr. at 1535 E. Hammer, Stockton. Ordered 2 <br /> hamburgers on 10/16/2015 at 4:30pm. sick with bloody diarrhea, nausea, and dizziness on 10/18/2015. 1 had bloody <br /> diarrhea for 3 days and I am still experiencing dizziness today. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately. (c) stated he had two$1.59 "Big Hamburgers" ordered from <br /> drive-through. (c) had breakfast (egg and sausage) on Saturday with roommate and Vietnamese soup (hot)with girlfriend for <br /> dinner in evening; neither roommate nor girlfriend had any symptoms. (c) came down with symptoms on Sunday (10/18) <br /> morning, almost fainting at a store at one point. Bowel movement about 3 times a day during the next 3 days. No fever nor <br /> sweating; no lab work was done to confirm if it is "E. coli". Advised(c)to seek medical attention as soon as possible. <br /> Interviewed manager, who stated: <br /> - not aware of this or any other similar complaints. <br /> - no employee or family members sick recently; last sick call was about 3 weeks ago. No injuries. <br /> - not aware of any employee working at other facilities <br /> -employee received food safety training <br /> -staff do eat food from facility and had not experienced any food symptoms <br /> - not aware of any employee travelled out of county. Employees live in Stocxkton <br /> Observed Big Hamburger put together: <br /> 1. Patties are grilled on both sides (flame boiled on bottom side). 184 F immediately out of conveyor grill. <br /> 2. Patties stored in hot hold: 167 F added to buns slightly (toasted). <br /> 3. Chopped lettuce (70 F) and sliced onions (64 F) added to buns. Time control is used for these items. <br /> 4. Pickles(non PHF) added. <br /> 5. No cheese nor mayo on this item <br /> Hand wash soap and towels available at kitchen and at restroom. Temperature of water 123, 127, and 128 F <br /> Storage of food appropriate and no signs of cross-contamination <br /> Sanitizing conducted.
