This information must be current and submitted as part of the business plan. <br /> Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, enter the <br /> correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class 11. <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br /> The site map was not complete as part of the business plan. Pleas list the locations of the gates, spill kit equipment, <br /> and the utilities shut offs. A site map shall contain a north orientation, loading areas, internal roads, adjacent <br /> streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment. If a site map element is not applicable <br /> for your facility then list it on the map and label as"NA". Immediately log into the California Environmental Reporting <br /> System (CERS)at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for <br /> approval. <br /> This is a repeat violation, Class II. <br /> 9 HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS)annually. The business plan was last submitted on 8/22/2018. Please make a submittal for 2019. <br /> The hazardous materials inventory shall be submitted by January 15 of each calendar year and may be submitted <br /> beginning November 1 of the previous year. Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the <br /> correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class 11. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 2/29/2019. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
