All components installed must be certified by an independent testing organization <br />and bear the appropriate mark as required by the independent testing <br />organization. <br />5. Complete and submit a copy of the Underground Storage Tank Certification of <br />Installation/Modification (Former Form C) for the regular (87) tank no later than 30 <br />days after work is completed. <br />8. This permit expires 180 days from the approval date. <br />9. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work.
