Complete and submit a copy of an Underground Storage Tank Certification of <br />Installation/Modification (Former Form C) to this office no later than 30 days after <br />work is completed. <br />7. This permit expires 180 days from the approval date. <br />8. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work.
