The maintenance records for the Q1: 91 PLLD alarms on January 8 and 9, 2013, and the Q2: 87 <br /> PLLD alarm on December 2, 2012 were not on site. The facility alarm log did not include the Q1: 91 <br /> PLLD alarms on January 8 and 9, 2013, and the Q2: 87 PLLD alarm on December 2, 2012. These <br /> records shall be kept on site for at least 3 years. Immediately document the UST monitoring system <br /> alarms on the facility alarm log, locate and maintain the maintenance record for these alarms on site, <br /> and submit copies to the EHD by February 15, 2013. <br /> 204. The designated operator failed to document all the alarms from the attached alarm history on the <br /> designated operator monthly inspection report and failed to check that they were responded to <br /> appropriately. The missing alarms include: Q1: 91 PLLD alarms on January 8 and 9, 2013, and the Q2: <br /> 87 PLLD alarm on December 2, 2012. During the monthly inspection, the designated operator shall <br /> review the alarm history for the previous month, check that each alarm was documented and responded <br /> to appropriately, and attach a copy of the alarm history with documentation taken in response to any <br /> alarms to the monthly report. Ensure that designated operators performing monthly inspections at this <br /> facility are including all of the required information on the reports. <br /> 204. The designated operator failed to check and note the correct test dates for the secondary <br /> containment on the January 11, 2013, designated operator monthly inspection report. The January 11, <br /> 2013, designated operator monthly inspection report lists the secondary containment testing as May 18, <br /> 2012. It should be listed as May 18, 2011. During the monthly inspection, the designated operator shall <br /> check that all required testing and maintenance for the UST system have been completed and document <br /> the dates they were done. Ensure that designated operators performing monthly inspections at this <br /> facility are including all of the required information on the reports. <br /> 204. The designated operator failed to verify that all employees have been trained during the January <br /> 11, 2013, monthly inspection. During the monthly inspection, the designated operator shall verify that all <br /> facility employees have been trained.
