The following is an itemized list of hazardous waste violations that have not been <br /> addressed for Caltrans-Lodi as of March 20, 2019. <br /> Open violations from August 08,2018 inspection <br /> Violation#114-Failed to keep signed copy of manifests from the designated facility for three years. <br /> Copies of uniform manifests for the disposal of hazardous waste from 2015 were not found on site. Hazardous <br /> waste generators shall retain copies of all manifests signed off by the disposal facility on site for three years and <br /> have them readily available for review. Immediately locate a copy of all manifests for the last three years, maintain <br /> them on site, and submit copies of the manifests from August 8,2015- December 31, 2015 to the EHD. <br /> Manifest numbers 005329414SKS(4/26/16)and 005329471 SKS(4/26/16)were not found on site. Hazardous <br /> waste generators shall retain copies of all manifests for three years or until a copy signed off by the destination <br /> facility is received. The signed off copy shall be retained and made readily available for review for three years. If <br /> the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the <br /> facility to which the generator's waste was submitted within 35 days of the date the waste was accepted by the initial <br /> transporter,the generator shall contact the transporter and/or the owner or operator of the designated facility to <br /> determine the status of the waste. If the generator did not receive a copy of the signed off copy of the manifest <br /> within 45 days of the date that the waste was accepted by the initial transporter,the generator shall submit an <br /> Exception Report that includes a legible copy of the missing manifest, and a cover letter signed by the generator <br /> explaining the efforts to locate the hazardous waste and the results of those efforts. Exception reports shall be <br /> submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br /> Immediately locate a copy of the missing manifests or prepare and submit an exception report to DTSC. Submit to <br /> the EHD a copy of the signed off copy of the manifests or a copy of the exception report submitted to DTSC. <br /> Manifest numbers 005180618SKS(1/27/16), 015493326JJK(4/4/16), 005453155SKS(7/12/16), 005728338SKS <br /> (11/8/16), 005631770SKS(2/14/17), 005742658SKS(5/11/17), 005862663SKS(1/19/17), 006136821SKS(8/2/17), <br /> 006363462(10/25/17),006531654SKS(1/19/18), 006325790(1/19/18), and 006520121SKS(4/23/18)were found <br /> without a signed off copy from the destination facility. Hazardous waste generators shall retain copies of all <br /> manifests signed off by the destination facility on site for three years and have them readily available for review.
