The following is an itemized list of hazardous waste violations that have not been <br /> addressed for Caltrans-Lodi as of March 20, 2019. <br /> Open violations from January 11,2016 inspection <br /> Violation#111 -Failed to comply with uniform hazardous waste manifest exception requirements. <br /> Manifest numbers 003629449SKS(1/17/13), 003681537SKS(1/17/13), 004054066SKS(3/11/14), 004061429SKS <br /> (12/17/13), 004132370SKS (3/11/14), 004334416SKS (8/26/14), 004634187SKS(2/9/15), 004672798SKS <br /> (11/17/14), 004919569SKS(5/11/15), and 005145071 SKS(11/6/2015)were found without a signed off copy from <br /> the destination facility. Documentation of notification to the Department of Toxic Substances Control for not having <br /> received a signed off copy of the manifests was not available for review. Hazardous waste generators shall retain <br /> copies of all manifests signed off by the destination facility on site for three years and have them readily available for <br /> review. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or <br /> operator of the facility to which the generator's waste was submitted within 60 days of the date the waste was <br /> accepted by the initial transporter,the generator shall submit a legible copy of the missing manifest,with some <br /> indication that the generator has not received confirmation of delivery. This information shall be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br /> Immediately locate a copy of the missing manifests or prepare and submit the required information to DTSC. <br /> Submit to the EHD a copy of the signed off copy of the manifest or a copy of the information submitted to DTSC. <br /> Manifest numbers 003923743SKS(8/21/13), 004354952SKS(6/4/14), 004956092SKS(8/12/15), 008952844JJK <br /> (4/30/13), and 009405316JJK(1/22/13)were not found on site.
