AN Environmental Health Department <br /> QU <br /> COUNTY— <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> BELKORP AG- STOCKTON 1120 W CHARTER WAY, STOCKTON February 08, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 5 HSC 25505(a)(1) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. <br /> One 300 gallon tank of waste oil and one 55 gallon drum of waste antifreeze were found on site that have not been <br /> reported. Any material that meets or exceeds the reportable quantity shall be reported in the facility's business plan. <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, add the <br /> materials to the hazardous material inventory, and submit to the EHD for approval. <br /> The Hazardous Materials Inventory diesel fuel, oxygen, and acetylene are not complete. The inventory form for <br /> diesel does not show the average amount, size of the largest container, and days on site, the inventory forms for the <br /> compressed oxygen and acetylene wrongly identify the storage pressure as"ambient" vs. "above ambient". This <br /> information must be current and submitted as part of the business plan. Immediately log into the California <br /> Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the correct or updated information, and <br /> submit to the EHD for approval. <br /> This was corrected on site. <br /> This is a repeat violation, Class II. <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br /> The site map does not show north orientation, loading areas, storm and sewer drains, access and exit points, <br /> hazardous materials and storage areas, emergency response equipment (fire extinguishers and spill control), <br /> emergency shutoffs, does not have a legend to identify the codes/symbols used on the map, and the map is not <br /> legible. Submit a corrected inventory and map. The hazardous materials inventory site map must contain the <br /> following information: north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br /> access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage <br /> areas and emergency response equipment. If a site map element is not applicable for your facility, then list it on the <br /> map and label as 'NA', or include a statement on the map that says the element is not on site. <br /> This is a repeat violation, Class II. <br /> 7 HSC 25505(a)(3) Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures were lacking. Section C. the phone number for the Local Unified Program <br /> Agency,209-468-3420 has not been identified. A business plan shall include the following emergency response <br /> procedures for a release or threatened release of hazardous materials, including, but not limited to, the following: <br /> - Immediate notification of local emergency personnel and the Environmental Health Department (EHD) <br /> - Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> - Evacuation plans and procedures, including immediate notice, for the site <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> This was corrected on site. <br /> This is a Class II violation. <br /> FA0009133 PR0519408 SCO02 02/08/2019 <br /> EHD 19-01 Rev.09/27/2018 Page 2 of 3 Hazardous Materials Program OIR <br /> 1868 E.
