The following is an itemized list of hazardous materials violations that have not been <br /> addressed for SRH FOOD & GAS as of March 20, 2019. <br /> Open violations from February 07, 2019 inspection <br /> Violation #6 - Failed to complete and/or submit a site map with all required content. <br /> The map does not show loading areas, adjacent streets, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas (gasoline and diesel), and emergency response equipment(fire extinguishers <br /> and spill control). A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm <br /> and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material <br /> handling and storage areas, and emergency response equipment. If a site map element is not applicable for your <br /> facility then list it on the map and label as"NA". Immediately log into the California Environmental Reporting System <br /> (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> Violation #9 - Failed to annually review and certify that the business plan is complete, accurate, and up to <br /> date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS) annually by January 15 . The hazardous materials inventory shall be submitted by January 15 of <br /> each calendar year and may be submitted beginning November 1 of the previous year. Immediately log into the <br /> CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> Violation #12 - Failed to have the business plan readily available to facility or EHD personnel. <br /> Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency <br /> response plans and procedures, the inventory of information required by this article, and the site map required by <br /> this section shall be readily available to personnel of the business or the unified program facility with responsibilities <br /> for emergency response or training pursuant to this section. Immediately make arrangements to have the business <br /> plan readily available for review. <br /> Open violations from March 22, 2016 inspection <br /> Violation #6 -Failed to complete and/or submit a site map with all required content. <br /> A site map was not submitted for 2016 and the site map submitted for the reporting year 2015, did not contain a <br /> north orientation, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation <br /> staging areas, propane storage area, and emergency response equipment. A site map shall contain a north <br /> orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, <br /> emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency <br /> response equipment.
