AN Environmental Health Department <br /> QU <br /> COUNTY— <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> SRH FOOD &GAS 749 E DR MARTIN LUTHER KING JR BLVD, February 07, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 5 HSC 25505(a)(1) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. <br /> Eighteen 15 pound cylinders (64 gallons)of propane were found on site that have not been reported. Any material <br /> that meets or exceeds the reportable quantity shall be reported in the facility's business plan. Immediately log into <br /> the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, add the materials to the <br /> hazardous material inventory, and submit to the EHD for approval. <br /> This is a repeat violation, Class II. <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br /> The map does not show loading areas, adjacent streets, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas (gasoline and diesel), and emergency response equipment (fire extinguishers <br /> and spill control). A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm <br /> and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material <br /> handling and storage areas, and emergency response equipment. If a site map element is not applicable for your <br /> facility then list it on the map and label as "NA". Immediately log into the California Environmental Reporting System <br /> (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class II. <br /> 9 HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS) annually by January 15 . The hazardous materials inventory shall be submitted by January 15 of <br /> each calendar year and may be submitted beginning November 1 of the previous year. Immediately log into the <br /> CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class II. <br /> 12 HSC 25505(c) Failed to have the business plan readily available to facility or EHD personnel. <br /> Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency <br /> response plans and procedures, the inventory of information required by this article, and the site map required by <br /> this section shall be readily available to personnel of the business or the unified program facility with responsibilities <br /> for emergency response or training pursuant to this section. Immediately make arrangements to have the business <br /> plan readily available for review. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, within 30 days of receiving the completed inspection report. <br /> Facility does not have internal roads and this element was not addressed on the site map. Please address this <br /> by either writing "NA" for not applicable or a statement that says the element is not on site.
