Both live roaches killed on site. <br /> Observed multiple dead cockroaches throughout the kitchen (on the wall behind the ice machine, in a bucket under the <br /> handsink, and on the floor under and behind cooking equipment). <br /> Observed one live adult German cockroach and multiple dead German cockroaches on glue boards throughout the facility. <br /> Immediately clean up dead roaches and clean and sanitize the areas affected. <br /> Facility contracts with Terminix Commercial. Observed invoices dated 11-20-18, 12-6-18 and 12-18-18(photo taken of <br /> invoice from 12-18-18. <br /> Facility owner called Terminix during inspection requesting an additional service. <br /> CALCODE DESCRIPTION:Each food facility shall be kept free of vermin:rodents(rats,mice), cockroaches,flies.(114259.1, 114259.4, <br /> 114259.5) <br /> #7 Hot and Cold Holding Temperatures <br /> OBSERVATIONS:The 2 door prep unit is 47F.
