Wilson, R.E.H.S. <br />Margaret Lagorio, R.E.H.S. <br />Robert McClellon, R.E.H.S. <br />Mark Barcellos, R.E.H.S. <br />The permit application for the exploration and repair of a secondary piping line at the above referenced <br />facility has been approved with the following conditions. <br />1. Notify Local Air pollution control and local Fire districts for all applicable permits. <br />2. Ensure all replaced parts are UL approved for the specific application. <br />3. Contractor shall be trained by product manufacturer for application/installation of all proposed <br />replacement products. <br />4. Ensure all replaced parts of the UST system are cathodicly protected. <br />5. Sampling may be required as per EHD assessment. <br />6. A tank integrity test may be required, by a third party certified tank tester, prior to the final <br />' inspection. <br />7. Primary and secondary lines will need to be inspected while under pressure with an inert gas, <br />prior to backfilling. A soap test will be required on the secondary line. <br />8. The piping slope will need to be inspected prior to backfilling. <br />9. Arrange all inspections 48 hours in advance. <br />10.
