The site map is missing a loading area, <br /> internal roads, adjacent streets, storm and sewer drains, accesses and exit points, evacuation staging area, <br /> emergency response equipment locations(fire extinguishers), utilities shut offs, and hazardous material storage <br /> area. Please add the missing elements to the site map. <br /> A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer <br /> drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and <br /> storage areas, and emergency response equipment. If a site map element is not applicable for your facility then list <br /> it on the map and label as"NA". Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 7 HSC 25505(a)(3) Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures were not complete. The emergency response and contingency plan is <br /> missing information on line A2 ,A3 , G, and in section J. On line A3 please enter in a date. Line A2 please list the <br /> California Environmental Reporting System (CERS) ID#is 10478197. Please change the phone number on line C3 <br /> to say 209-468-3420. Please list all of the locations of the marked boxes in section G. <br /> A business plan shall include the following emergency response procedures for a release or threatened release of <br /> hazardous materials, including, but not limited to,the following: <br /> -immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> -procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> -evacuation plans and procedures, including immediate notice,for the site <br /> Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> 9 HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS)annually.The business plan was not submitted between 11/1/2018- 1/15/2019.The business plan <br /> was submitted on 2/26/2019 making the plan late. <br /> The hazardous materials inventory shall be submitted by January 15 of each calendar year and may be submitted <br /> beginning November 1 of the previous year.
