The Hazardous Materials Inventory forms for used oil were not complete. The used oil needs the new federal hazard categories on the inventory sheet. This information must be current and submitted as part of the business plan. Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. This is a minor violation. 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. The site map was not complete as part of the business plan. The site map was not viewable.Te site map must be uploaded as a PDF. A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and label as"NA". Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. This is a minor violation. 7 HSC 25505(a)(3) Failed to establish and/or submit emergency response procedures for a release or threatened release. The emergency response procedures were not complete. The emergency response procedures is missing information on line1 and Al. Please put a date for line Al. Please put 10187351 as the CERS ID#on line 1. A business plan shall include the following emergency response procedures for a release or threatened release of hazardous materials, including, but not limited to,the following -immediate notification of local emergency personnel and the Environmental Health Department(EHD) -procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to persons, property, or the environment -evacuation plans and procedures, including immediate notice,for the site Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. This is a repeat violation, Class II. 9 HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to date. The business plan information has not been reviewed and resubmitted in the California Environmental Reporting System (CERS)annually.The last submittal made was on 7/21/2017. Please submit all three parts of the business plan for 2019. The hazardous materials inventory shall be submitted by January 15 of each calendar year and may be submitted beginning November 1 of the previous year. Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. This is a Class II violation.
