The Hazardous Materials Inventory form for propane was not complete. Propane max daily amount is listed at 10,685 cubic feet. Please update the max daily to read as 10,685 gallons of propane on site. This information must be current and submitted as part of the business plan. Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. This is a minor violation. 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. The site map was not complete as part of the business plan. The site map is missing emergency shut offs propane, generators, utilities) locations, emergency response equipment(fire extinguishers, spill kits)locations , all hazardous material storage areas( propane, diesel, gasoline)locations, storm drain locations, evacuation staging area locations, access and exit points.A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and label as"NA". Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. This is a minor violation. 9 HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to date. The business plan information has not been reviewed and resubmitted in the California Environmental Reporting System (CERS)annually. The Hazardous Material Business Plan (HMBP)was not submitted between 11/1/2018- 1/15/2019.The HMBP was submitted late on 1/22/2019.The hazardous materials inventory shall be submitted by January 15 of each calendar year and may be submitted beginning November 1 of the previous year. Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. This is a Class II violation. 15 HSC 25510(a)Failed to report actual or threatened release to the EHD and/or CaIOES Warning Center. A release of diesel occurred on 1/23/2019 and was not reported to the EHD and CaIOES. The diesel fittings or hose is leaking. Please stop the leak and clean up the spill. A release or threatened release of a hazardous material shall be verbally reported immediately to the EHD and CaIOES. Ensure that any future releases or threatened releases of a hazardous material are verbally reported immediately to the EHD and CaIOES. This is a Class I violation. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by 2/22/2019.
