<br /> 5 HSC 25505(a)(1) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site.
<br /> A 275 gallon tote of asphalt coating solution but, labeled UN3267, Corrosive Liquid, Basic, Organic, N.O.S., was
<br /> found on site that have not been reported. Any material that meets or exceeds the reportable quantity shall be
<br /> reported in the facility's business plan. Immediately log into the California Environmental Reporting System (CERS)
<br /> at http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for
<br /> approval.
<br /> The Hazardous Materials Inventory forms for the compressed oxygen, argon compressed, soluble oil, acetylene,
<br /> liquefied petroleum gas, and compressed carbon dioxide gas were not complete. The storage pressure for
<br /> compressed oxygen, argon, acetylene, and liquefied petroleum gas has wrongly been identified as "ambient"vs.
<br /> "above ambient". The days on site for compressed oxygen, soluble oil, and carbon dioxide has not been identified.
<br /> The storage temperature for carbon dioxide has not been identified. The size of the largest container, average daily
<br /> amount, days on site, storage pressure, and storage temperature for the compressed nitrogen have not been
<br /> identified. This information must be current and submitted as part of the business plan. Immediately log into the
<br /> California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the correct or updated
<br /> information, and submit to the EHD for approval.
<br /> This is a repeat violation, Class II.
<br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content.
<br /> The map does not show hazardous material handling and storage areas (asphalt coating solution). A site map shall
<br /> contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit
<br /> points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and
<br /> emergency response equipment. If a site map element is not applicable for your facility then list it on the map and
<br /> label as "NA". Immediately log into the California Environmental Reporting System (CERS) at
<br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval
<br /> This is a Class II violation.
<br /> 7 HSC 25505(a)(3) Failed to establish and/or submit emergency response procedures for a release or threatened
<br /> release.
<br /> The emergency response procedures were lacking. Section C. the phone number for the Local Unified Program
<br /> Agency,209-468-3420 has not been identified. A business plan shall include the following emergency response
<br /> procedures for a release or threatened release of hazardous materials, including, but not limited to, the following:
<br /> - Immediate notification of local emergency personnel and the Environmental Health Department (EHD)
<br /> - Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to
<br /> persons, property, or the environment
<br /> - Evacuation plans and procedures, including immediate notice, for the site
<br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the
<br /> correct or updated information, and submit to the EHD for approval.
<br /> This is a repeat violation, Class II.
