N.JOAQUIN Environmental Health Department <br /> COUNTY <br /> t;reocnt?ss grows Akers. <br /> Time In: 2:05 pm <br /> Time Out: 2:40 pm <br /> Food Program Complaint Inspection Report <br /> Name of Facility: S-MART#334 Date: 05/18/2017 <br /> Address: 3215 PACIFIC AVE, STOCKTON 95204 <br /> Owner/Operator: SAVE MART SUPERMARKETS Telephone: (209)464-9431 <br /> Program Element: 1600-FOOD PROGRAM Complaint#: C00043466 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> Complainant purchased medium size watermelon either on 05/09 or 05/10/2017. complanant and spouse ate some of the <br /> melon on friday afternoon on 5/12/17.Approximately 10 pm both became ill with symptoms of cramping, gas, diahhea, <br /> chills, body ache, and exhaustion that continued through Sunday. Complainant stated that the watermelon is the only thing <br /> they believe made them sick. Prior to getting sick both the complainant and spouse consumed homemade spagetti. Spouse <br /> consumed spagetti again on 05/14/17 and did not become ill. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> Spoke to store manager, Forest who was not aware of this complaint or any other complaint regarding the watermelons. <br /> Observations: Melons are kept on large bins.
