F,y
The wells planned for destruction are wells MW-1,MW-2,and M -3 located on the service.station property. A site
map and well boring diagrams are attached. The wells are propose d to be destroyed using the pressure grouting
method by Gregg Drilling,Inc.,a,licensed drilling contractor: Nez t cement grout will be pumped into the well
casing under pressure greater than 25 psi and held for more than fi e minutes to force grout through the screen
interval and adjacent sandpack materials and effectively seal the entire depth of well casing and sandpack. The wells
will be inspected and any obstruction removed, from the well casing prior to pressure grouting.
After pressure grouting each well, the surface well'box will be removed. The top 3-feet of well borehole will be over
drilled using 10-inch diameter augers, and approximately the top 2.5-feet of well casing will be removed. The
bottom of the overdrill-borehole will be filled with approximately ne-foot of cement to establish a cement base and
a 2.5-foot bgs"mushroom cap". The cap will be covered with clean fill and the ground surface at each well location
will be restored to surface grade using replacement materials(i.e., asphalt or cement patch).
Following completion of well destruction activities,Azure-will submit a report documenting completion of such
activities to the SJCEHD.
